Yes, self-pay is accepted as below;

  3. No-show or Late cancellation
    • $75
    • Cancellation must be a day before your scheduled appointment to avoid a $75 fee.

Yes, your mental health services can be offered to you remotely or virtually through a secured private video/audio technologies through the internet at your chosen location and convenience. You will be able to access quality personalized care from a cellphone, tablet, or computer.

Yes. During or after your telehealth visit, the provider will electronically prescribe and send medications to your preferred pharmacy. Please note that controlled substances, such as benzodiazepines (Xanax, Ativan, Valium, Klonopin, etc.) and stimulants (Adderall, Vyvanse, Ritalin, Focalin, Concerta, etc.), will not be prescribed. Our providers will carefully assess and prescribe the most appropriate evidence-based medications based on your clinical needs.

These are the services we do not provide:

  • We do not conduct Social Security Disability Insurance (SSDI) psychiatric evaluations for patients who have been with us for less than 6 months.
  • We do not provide worker’s compensation evaluations.
  • We do not treat severe eating disorders.
  • We do not offer primary care services.
  • We do not provide pain management or prescribe opioid medications. Additionally, we are unable to refill opioid prescriptions for current patients if their prescriber is unavailable.
  • We do not prescribe benzodiazepines to patients who are currently taking opioid medications, carisoprodol, or methadone.
  • We do not provide crisis services. If you or someone you know is experiencing a psychiatric emergency, please do not contact our office; instead, call 911 or 988. You can also visit an Emergency Room for urgent or crisis situations, where they accept walk-ins and provide appropriate care without requiring an appointment.

We provide services to the following age groups:

  • Adults (ages 18 to 64).

Our psychiatry medication management services are only available across the state of Massachusetts.

What types of forms or documentation does LifeTent not complete or sign?

LifeTent Behavioral Health, LLC does not complete, sign, or provide forms or evaluations that require forensic determination, legal adjudication, independent medical examination, public-safety certification, or occupational fitness assessment.

This includes, but is not limited to:

  • Forensic evaluations or legal opinions

  • Custody or parenting capacity assessments

  • Fitness-for-duty certifications

  • Return-to-work determinations

  • Department of Motor Vehicles (DMV/RMV) medical clearance forms

  • Driving fitness evaluations, including those related to DUI or substance-related license actions

  • Firearms-related evaluations, certifications, or reinstatement documentation (including License to Carry [LTC] or Firearms Identification [FID] requests)

  • Emotional Support Animal (ESA) letters or related paperwork

  • Social Security Disability (SSI/SSDI) evaluations or disability determinations

  • Court-ordered, law-enforcement, or employer-mandated medical certifications requiring independent assessment.

Why can’t LifeTent complete these types of forms?

These requests typically require specialized evaluations, collateral investigations, legal or forensic determinations, or public-safety assessments that cannot be appropriately or ethically completed within an outpatient psychiatric treatment relationship.

Are there any forms or documentation LifeTent may complete?

Yes. Certain documentation may be considered on a case-by-case basis, depending on clinical appropriateness, sufficient evaluation, medical necessity, and available supporting information. These may include:

  • Family and Medical Leave Act (FMLA) paperwork

  • Leave of Absence (LOA) forms

  • School accommodation documentation

  • Workplace accommodation documentation

  • Housing or accessibility evaluations

Completion of these documents is not guaranteed and may require additional appointments.

Can I request copies of my records or a treatment summary?

Yes. Upon request and with proper authorization, LifeTent Behavioral Health, LLC may provide copies of medical records or a treatment summary, which may include diagnoses, dates of service, treatment plans, and progress notes.

Please note that medical records and treatment summaries do not constitute legal, forensic, occupational, fitness-for-duty, or disability determinations.
