Introduction

These Terms of Service (“Terms”) govern the use of the psychiatric mental health outpatient services provided by LifeTent Behavioral Health, LLC (“Provider,” “we,” or “us”) to the individuals (“Clients” or “you”) seeking mental health treatment. By using our services, you agree to abide by these Terms.

Service Description:

a. LifeTent Behavioral Health, LLC provides psychiatric mental health outpatient services to individuals seeking professional assistance for mental health conditions.

b. Our services may include psychiatric evaluations, medication management, therapy sessions, counseling, and other related mental health services.

Client Responsibilities:

a. Provide accurate and complete personal and medical information during the intake process.

b. Attend scheduled appointments promptly and adhere to cancellation policies.

c. Participate actively in your treatment plan, follow the Provider’s recommendations, and communicate any concerns or changes in your condition.

d. Make payments for services rendered in accordance with the Provider’s fee structure and payment policies.

Provider Responsibilities:

a. Provide professional psychiatric mental health services consistent with industry standards.

b. Maintain the privacy and confidentiality of client information in accordance with applicable laws and regulations.

c. Create an individualized treatment plan based on the client’s needs and goals.

d. Communicate any changes to appointment schedules or treatment plans in a timely manner.

e. Respect the client’s rights, autonomy, and cultural values throughout the treatment process.

Privacy and Confidentiality:

a. Client information, including personal and medical records, will be treated as confidential and will not be disclosed to third parties without the client’s written consent, except as required by law.

b. The Provider will take reasonable precautions to protect the confidentiality and security of client information.

Termination of Services:

a. Either party may terminate the services by providing written notice to the other party.

b. The Provider reserves the right to terminate services if the client engages in behavior that poses a threat to themselves, or others, or if the client violates these Terms.

Intellectual Property:

a. All materials provided by the Provider, including but not limited to treatment plans, therapy notes, and educational resources, are protected by intellectual property laws.

b. Clients may not reproduce, distribute, or modify any materials without the Provider’s written consent.

Limitations of Liability:

a. The Provider’s liability for any claims arising from the services provided is limited to the fees paid by the client for the specific services.

b. The Provider shall not be held responsible for any indirect, incidental, or consequential damages arising from the use or inability to use the services.

Governing Law and Jurisdiction:

These Terms shall be governed by and construed in accordance with the laws of Massachusetts. Any disputes arising out of or in connection with these Terms shall be subject to the exclusive jurisdiction of the courts in Massachusetts.

Modification of Terms:

The Provider reserves the right to modify these Terms at any time. Updated Terms will be made available on our website or provided to clients directly.

By using our psychiatric mental health outpatient services, you acknowledge that you have read, understood, and agreed to these Terms. If you have any questions or concerns, please contact our office for clarification before proceeding with treatment.

We accept new patients

Yes, we accept new patients in Massachusetts. Our availabilities are Monday to Friday: 7 p.m. to 10 p.m., and Saturday: 9 a.m. to 10 p.m. We have an immediate appointment, but you will be given 48 hours to fill out intake documentation before your initial consultation.
