Process
Insurance Verification: Upload below or send/text clear photos of the front and back of your current insurance card to (508) 635-0144. Failure to do so may result in delays in verifying your insurance eligibility and scheduling your appointment.

1: Submit Your Request
- Use the Book Appointment button to request an appointment. Most appointments are available within 48 business hours.
- Please remember to upload or send clear photos of the front and back of your insurance card so we can verify your insurance coverage.
2: Insurance Verification & Scheduling
- We verify your insurance eligibility before scheduling your appointment. Once your insurance has been verified, your appointment will be scheduled, and your intake forms, along with a link to create your patient portal account, will be sent to you by text message and email (please check your spam or junk folder if you do not see them). Please complete and submit all required forms online before your scheduled appointment.
- All required intake forms must be fully completed and submitted online at least 4 hours before your scheduled appointment. Failure to complete and submit all required paperwork by this deadline may result in your appointment being canceled or rescheduled.
- If your appointment is canceled due to incomplete forms, you may reschedule it through the patient portal, which can be accessed on this website.
3: Meet Your Provider
- You will receive appointment reminders by text message and voice message before your scheduled appointment. The text message reminder will include a secure link to join your telehealth visit.
- Attend your scheduled appointment for a comprehensive psychiatric evaluation and personalized treatment planning.
